Registered Nurse (RN) or Licensed Practical Nurse (LPN) provides skilled nursing care in the home, in accordance with the physician's signed plan of treatment. The RN is responsible for the initial evaluation visit and establishes a Plan of Care and regularly reevaluates the nursing needs of the individual. Nurses have expertise in geriatric care, IV infusion, diabetes, cardiology, wound care, pain management, oncology, Alzheimer's and chronic disease management.
