Intensive Care Coordination (ICC) Services, Children and Adolescents

Henrico Area Mental Health and Developmental Services (HAMHDS) Intensive Care Coordination (ICC) Services uses the High Fidelity Wrap Around Model to maintain youth in a home-like setting. Youth who have been involved with significant behavioral health intervention and who are at risk of being placed at a residential facility can be referred for this service. The ICC coordinator works with the family to identify supports, services and develop a detailed crisis intervention and prevention plan. The ICC coordinator facilitates monthly family and team meetings and provides support to the family insuring their needs are being met by service providers.
