Neighborhood Health, South County Center

Neighborhood Health (formerly Community Health Care Network (CHCN)) has multiple clinical sites in the city of Alexandria and the counties of Arlington and Fairfax.  This location provides primary care for women and adults.

Primary care provides 

• Chronic disease management
• Routine annual checkups
• Urgent care appointments
• Onsite laboratory testing
• Electrocardiograms (EKGs)
• Routine adult vaccinations
• Diabetes education
• Cancer screenings
• Referral Coordination
• Urgent after-hours consults

Our mission is to improve health and advance health equity in Alexandria, Arlington, and Fairfax by providing high-quality primary care regardless of the ability to pay.    Interpreter services are available.
