Who We Are
The Wellness Passport offers free preventive health services. Visits for $30 with Blue Ridge Medical Center providers, and assistance with enrollment processes for sliding scale programs offered by University of Virginia Health System (Charlottesville), Martha Jefferson Hospital (Charlottesville), Centra Health (Lynchburg) and Augusta Health Care (Fishersville).
To qualify for the passport, individual must not have health insurance and must have certain income levels, depending on size of family.
