Who We Are
The Montgomery County Health Department offers a pre-admission screening process for individuals seeking Cardinal Care Medicaid coverage of long-term care services. Requests for screenings, however, need to be made through the Department of Social Services.
An in-home assessment is done by a registered nurse and a social worker and referrals for other community-based resources are made. Contact your local Department of Social Services for an appointment.
