Who We Are

Hazelwood House, an Adult Day Services Care facility, is a comprehensive program of services and opportunities for older adults with special needs. The Center offers a warm and positive approach that provides socialization, mental stimulation and exercise appropriate for physical capabilities of participants. Offers daytime nutrition. Certified health care professionals are present at all times to monitor health-related needs of participants. Capacity is 50.



Transportation to and from Hazelwood House can be arranged through the Lake Country Area Agency on Aging's Transportation Department. Wheelchairs can be accommodated.
