Who We Are

At The Mount Rogers Community Services Board, the PACT (Program for Assertive Community Treatment) is a mobile multidisciplinary team that provides intensive clinical treatment, rehabilitation, and support services to individuals experiencing severe and persistent mental illness. This team provides psychiatry, case management, counseling, nursing, vocational, and substance addictions services in the individual's home or other community settings. These services are available seven days a week, 24 hours a day, and 365 days a year on a long-term basis. The emphasis is to provide intensive wrap around services that allow the individuals to remain stable in the community and avoid psychiatric hospitalization. Currently, services are limited to individuals residing in Wythe and Smyth counties.

Referrals may come from any source: The Mental Health Center, Psychiatric Facilities, IDC, Residential Services, Social Services, Case Management, friends and Family.
