Community Residences provides residential services to consumers who have Intellectual Disabilities and who need assistance and training to maintain and learn skills related to activities of daily living and use of community resources. Consumers have Support Coordinators assigned by the Community Services Board (CSB) who are active in their treatment and who support the consumer in developing a person-centered plan of care that best meets his or her individualized needs. The Program Manager, Counselors, Aides, Nurse and other people important to the consumers are all active members of the consumers’ team. Each group home is unique and serves consumers with a wide range of skills and needs. Funding for consumers in these programs comes through Medicaid Waiver, county contracts or private pay. The homes are located in the communities of Arlington and Springfield.
