Homeless Health Care, East End Physicians

Southeastern Virginia Health System operates East End Physicians, a community health clinic for the Homeless program (HCH). This program was established to provide health care to homeless individuals in Hampton and Newport News. Case managers are available to assist with services that are free to eligible participants.



Medical care to homeless patients is provided by a physician, a physician's assistant, or a family nurse practitioner. If you or someone you know is homeless and in need of medical care and case management services, please call 757-380-8709 to make an appointment.  

The HCH office is located in the Teagle Building at 1228 Warwick Boulevard at Suite 2D in Newport News, Virginia.
