Who We Are
Rappahannock Area Community Services Board (RACSB) operates five outpatient clinics located throughout Planning District 16, where behavioral health care encompasses more than therapy appointments and medication. Behavioral Health Case Managers help individuals navigate an array of treatment options, behavioral healthcare programs and community resources. Case Managers coordinate care to make sure individuals receive the appropriate treatment and customized care. They also connect individuals with available community resources, such as Social Security benefits, housing assistance, vocational training, employment and healthcare. Case Managers monitor treatment to help individuals on their path to behavioral health wellness. They review treatment plans and assess goals to make sure individuals receive the highest quality of services.
