Who We Are
Veteran Directed Care (VDC) program is a veteran’s directed home and community-based service program that serves eligible veterans of any age who are at risk of nursing home placement and their family caregivers. Services may be able to provide help with transportation expenses. Other services can include case management, adult daycare, assistive devices, caregiver support, carematch, environmental support, housing, home modifications, in-home personal care, and meal delivery.
VDC allows veterans to receive home and community based services that enable them to avoid institutionalization and continue to live in their homes and communities for as long as possible. Eligible veterans manage their own flexible budgets, decide what mix of goods and services best meet their needs, and hire and supervise their own workers.
