Who We Are
The Brunswick County Health Department, in collaboration with the local Department of Social Services, can assist with screening for individuals in need of nursing home placement. For Medicaid or Medicaid-eligible individuals, screenings are done by a team consisting of a nurse and a social worker. Determination is made to see if the individual meets the criteria for nursing home placement, personal care services in the home, or if some other community agency can best meet the individual’s needs. To have a screening, contact your local Social Services Department.
